 Service App Service Application – In order to be in compliance with the Federal Trade Commission’s Red Flags Rule, MEWCO requires all new customers to complete a Service Application and show state issued photo identification to one of our Customer Service Representatives.  We appreciate your assistance in helping to fight identity thief, and stopping criminals from using proof of utility service to get driver’s licenses illegally or apply for government benefits using a bogus address.  If you have any questions please call our Customer Service Department at 928-4550, Monday through Friday, 7:30 am to 4:30 pm. Lobby hours are Monday through Friday, 7:30 am to 4:00 pm. We are closed on holidays.
